TMS worked wonders for me! It was what finally got me over the last hurdle in my path. It's MUCH less invasive than ECT and there are no real side effects. TMS uses magnetic pulses to stimulate part of your brain, where as ECT uses jolts of electricity.
TMS isn't painful and is an outpatient treatment. I highly recommend it.

I would love to try TMS if I could afford it. I've been on countless meds, different combos, and had 14 rounds of ECT treatments and haven't really seen results. In my case (This is only my case, remember) the ECT treatments were difficult to deal with and didn't help.
I'm sure others have had success with ECTs, but not me.
The point made about Stage 4 sleep and Serotonin is interesting though because I've struggled with Insomnia and frequent awakening from sleep (like every 1- 2 1/2 hrs) I did have a Sleep Test in the hospital, but they could only do 1 day of testing. The test showed that I began REM in the last 10 mins before they were supposed to wake me up. I suppose that would cause some major problems with my Serotonin. I knew sleep played a major role as far as mental health goes- but that gives me something to look into. Because my sleep problems have lasted since childhood (regardless of schedule, meds, etc.) I believe I have a sleep-phase disorder of sorts. But I'm no Doctor!
