I just wanted to say I have a story like the bad physiatrist that I thought I'd mention. I was (again) being put through CBT via the NHS here in the UK. The second time I met my counsellor he called me Michael for the whole session (that's not my name). A friend kinda forced me to challenge him on it in the next session. He claims it was a mistake cos he read my name without his glasses on (my surname begins with m and is about the same length). I still don't believe him, he just had no idea who I was and didn't care.
